How to Use Your Insurance for Massage Therapy

How to Get Started:

For NYSHIP (Empire Plan) and AETNA clients:

● A prescription from a qualified provider (MD, PA, or Chiropractor) that includes:
● Your full name and date of birth
● A diagnosis code or description
● Must be written specifically for Massage Therapy (not Physical Therapy)
● Prescribing start and end dates
● Provider’s contact info and signature

● A copy of your insurance card (front and back)

For No-Fault and Workers’ Compensation cases:

● A prescription or referral
● Insurance claim number and adjuster’s contact information
● Legal representation details (if applicable)
