If you have any of the following plans, then your massage therapy sessions will be partially or fully covered:
- NYSHIP (Empire Plan)
- AETNA
- FSA (Flexible Spending Account) Cards
- HSA (Health Savings Account) Cards
What You'll Need to Use Insurance
Booking an appointment with Naturally Zuri requires a prescription from a qualified provider. Examples of qualified providers include your physician (MD), a physician’s assistant (PA-C), or a chiropractor. The prescription must include important details such as:
- Prescriptions for massage therapy, not physical therapy, must be written expressly.
- Your full name and date of birth.
- Contain a diagnosis code or condition.
- List the start and end dates for prescriptions.
- Provider contract details and signature.
A copy of your insurance card, both front and back, must be included with the prescription.
For HSA/FSA payments, you must:
- Use your HSA or FSA debit card at checkout.
- Services must be medically necessary (i.e., for pain, stress, recovery, etc.).
